Credit Application
At this time, your purchase order may be in the initial stages of production. However, we will not be able to continue in our production process without determining your credit status. A credit investigation will allow us to establish this statue and determine the terms of your account.
To that end, please complete our credit application and email it to contact@chicagotag.com at your earliest convenience.

If we have any questions regarding your application, we will be in contact with you. Otherwise, you will have an open account with thirty-day terms and the option of a discount of one percent for invoices paid within fifteen days.

Please note - Illinois Shipping Destinations for Distributors
If you would like us to ship your order to an Illinois destination or if you will be picking up your product at our dock, we must have a copy of your Illinois Resale Certificate. If you do not have an Illinois Resale Certificate, your invoice will include the applicable Illinois sales tax.
Otherwise, you can fill out a Certificate of Resale Blanket Form. By completing this form and submitting a copy of your Reseller's Certificate of Registration for Illinois, you can avoid being charged Illinois Sales Tax.
